DCFS Referral Process

To make a placement referral to Connected Foster Care, follow the steps below in order for us to complete placement. If you need assistance with this process, call our Connected On Call Number at 501-502-0647.

STEP ONE

If you have a referral for placement, submit the following referral form:

STEP TWO

Once you have submitted the referral, our Connected On Call Worker will contact you after checking available openings.

STEP THREE

If the referral is accepted for placement, please fill out the Placement Packet with e-Signatures and email to our Connected On Call Worker prior to placing the child. Our Connected On Call Worker will coordinate time of placement with our foster family.

Our staff serves as the point of contact for our families from referral and placement throughout the case process.
